Build a custom program form
Administrators can build electronic forms for processes such as weekly summaries, phase advancement, self-evaluations, and release from training. A form’s purpose and allowed starter roles determine how it is used in the program.
Start a new form
- Open Admin → Training Programs.
- Select the intended program’s name or Settings.
- Open Forms and start a new form.
- Enter the form name and choose its purpose.
- Select the roles allowed to start it, such as FTO, trainee, supervisor, or program administrator.
- Add a description and any instructions users should read.
- Set the active status and create the form.
Use the purpose that matches the process. A custom purpose is available when the listed purposes do not fit.
Add fields
Choose field types that match the answers you need. Available types include:
- Short text and paragraph responses.
- Dates and numbers.
- Dropdowns, multiselects, radio buttons, and checkboxes.
- File uploads.
- Section headings and informational text.
For choice fields, enter the options users should select from. Use headings to group related questions and informational text to explain what users should document.
Arrange and save the form
- Review the form preview alongside its field settings.
- Move fields using the ordering controls or drag and drop.
- Check the wording, choices, and instructions.
- Select Save Form.
Test the form
Open the Forms hub using a role allowed to start the form. Complete a practice submission and review the resulting layout, answers, and configured approval workflow.
If the form is missing, check its program, active status, and allowed starter roles.
Phase advancement forms
For a phase advancement form, select the phase-advancement purpose. Review the program’s Phase Advancements settings, sync transitions from phases when needed, and assign the intended form to each transition.
Test the transition and its approval chain with a practice trainee before using it for active training.
Edit a form already in use
A form can be deleted through the builder before it has been used. Once used, the builder provides editing rather than deletion.
Before changing an in-use form, review existing submissions and ask support how the proposed changes affect those records if that is unclear.
