Create a DOR and document daily training
A daily observation report, or DOR, brings together a trainee’s daily training information, assessments, narratives, and events. The fields and labels you see depend on your agency’s training model and program settings.
This guide describes a DOR started by an authorized FTO or staff member. PTO programs may use trainee-started Journals and a Ready for FTO handoff. Follow your program’s process for who starts the daily record.
Start a new DOR
- Confirm the correct active trainee and program on Dashboard.
- Select New DOR.
- Review the day number and the available week or phase fields.
- Select the shift and location, beat, or position your program uses.
- Check the hours worked and adjust any suggested hours to reflect the actual day.
- Select Create DOR.
If header details need correction while the record is editable, use the pencil control where offered.
Record assessments
If daily assessments are enabled, review the rating scale and category definitions before selecting ratings. Use the scale link, category information controls, or Definitions option where available.
Select the rating that matches the training documented. Rating selections save immediately in the demonstrated DOR editor. Add comments where required or helpful. Use N.O. or NRT only when offered and in accordance with your program’s definitions.
Some programs use event-level assessments, phase-specific assessment settings, or other methods. Complete the assessment areas shown for your program.
Write the daily narrative
Describe the training and activity for the day in the narrative fields provided. Complete any additional prompts, such as recommended training focus, when your program includes them.
If you insert a template, review and customize the wording. Use the applicable save controls or confirm the save status before leaving; do not assume every field uses the same save behavior as a rating selection.
Add an event
- Open the Events tab or event area.
- Select Add.
- Complete the fields shown, such as nature, participation level, reference, and narrative.
- Add category assessments, training time, or attachments when your program uses them.
- Select Save to add the event.
Add further events as needed. Some programs display events alongside daily assessments rather than on a separate tab.
Attach supporting files
Use the DOR’s file area for attachments supporting the daily record. When a file relates to a particular event, use that event’s attachment area where available. Confirm that the file is attached to the intended record.
Review and submit
Before signing and submitting, review the trainee, header details, assessments, required comments, narratives, events, time, and attachments. Create your saved signature in My Profile if required.
Use the DOR’s signing and submission action to begin its configured approval workflow. Submission changes the record’s editing availability; corrections should follow your agency’s return or reopening process.
