Complete a form and follow its approval workflow

Forms document processes such as weekly summaries, phase advancement, performance improvement plans, and program completion. Your agency chooses the forms available, who may start them, and the approval steps they follow.

Start or resume a form

  1. Staff: confirm the correct active trainee on Dashboard.
  2. Open Forms. Your trainee view may use a label such as My training forms.
  3. Find the form you need and review its purpose.
  4. Select Start, or open an existing draft from the form activity shown on the page.

If the form is missing, ask your administrator to check whether it is available in your program and whether your role is allowed to start it.

Complete the form

Read the instructions and complete the fields. Depending on the form, these may include text, dates, selections, attachments, or signatures.

If Insert Template is available, you can use a template as a starting point. Review the resulting text and adapt it to the trainee and situation.

If you need to return later, use Save draft where offered. Before submitting, review the full form and confirm its trainee, program, and any proposed phase transition. Create your saved signature in My Profile if signing is required.

Submit the form

Use the signing or submission control shown on the form, such as Sign & Submit or Submit for approval. This starts the workflow configured for that process.

After submission, use the displayed workflow status to see who is responsible for the next step. If the form is no longer editable, follow the correction process rather than starting a duplicate form.

Review an assigned form

  1. Open the form assigned to you.
  2. Read its content and any workflow comments.
  3. Review the current workflow step and the actions available to your role.
  4. Use the appropriate action, such as approval, acknowledgment, or return for corrections.

A form may follow a different approval chain from a daily training record.

Correct returned work

If a form is returned to you, read the return note, make the requested changes when editing is available, and resubmit using the displayed action. Contact your administrator if the status or available actions do not match what you expected.

Find historical forms

Open Reports and choose the report for the type of form you need, such as a performance improvement plan, weekly summary, or end-of-phase form. Open the submission to review its content and workflow history. Use Download PDF when available to save a copy.
