Working with forms

Who this is for

FTOs, supervisors, program admins, and trainees. Forms handle phase advancement, evaluations, release paperwork, PIPs, and custom agency processes.

Forms hub

Forms in the sidebar (when enabled).

  • Staff: select Active trainee on Dashboard first.
  • Trainee: My training forms for your enrollment.

Start a form

Start a form cards show name, purpose, description, and Start.

Which forms appear depends on allowed roles (trainee, FTO, supervisor, program admin) configured on each form.

Staff may also see Advance [phase]Complete program, or phase advancement alerts on the hub.

Complete a submission

  1. Fill fields (text, dates, dropdowns, signatures).
  2. Signatures use My Profile.
  3. Long text may offer Insert template….
  4. Save draft to continue later.
  5. Submit for approval or Sign and Submit for Approval when ready (signature required).

Phase advancement forms show which phase transition will occur when approved.

After submit

Fields become read-only. Approvers see workflow actions:

  • ApproveAcknowledgeSign and resubmit
  • Return to prior approver
  • Returned for corrections — edit draft and resubmit

Track your queue: Reports → Workflow status with My queue.

Recent activity

Hub table: FormActivityStatusOpenDelete (drafts only).

Statuses include DraftSubmittedIn reviewApproved.

Building forms (admins)

Configure under Program → Forms, templates, and Program → Workflow. See your agency admin or DORWORKS support for setup help.
